CDL Physical & DOT Medical Exams

Take your DOT physical for your CDL in our office near downtown Klamath Falls
Ready to get your Commercial Driver’s License (CDL)? Need to keep your CDL current?
Then you’re going to need a Department of Transportation (DOT) medical card. And to get that, you’ll need a DOT physical.
Dr. Smith is registered with the Federal Motor Carrier Safety Administration (FMCSA) to provide ODOT and CDL physicals for intra- and interstate truck drivers.
On the day of your DOT physical bring the following items:

- Current photo ID (required)
- Prescription eyeglasses if necessary
- A list of current medications
- Most recent Medical Examiner’s Certificate (MCSA 5876) if available
- Certain conditions will require additional information or physician clearance:
- Diabetes will require a recent, no older than 6 months, A1C
- If you are under the care of a cardiologist the underlying condition will require clearance by your cardiologist
You do not need to have a commercial drivers license to obtain medical certification which is commonly used for a preemployment physicals.
